Orbital Cellulitis

Snap Shot
  • orbital cellulitisA 29-year-old woman presents to the ED with a swollen and erythematous eyelid and an inability to adduct or abduct her eye.
Introduction
  • An infection of the orbit as a result of
    • untreated blepharitis
    • complication of paranasal sinus infection
    • facial trauma
     
  • Most common organisms include H. influenzae, S. pneumoniae, Staphylococcus, or Streptococcus
Presentation
  •  Symptoms 
    • eye pain
  • Physical exam 
    • opthalmoplegia
    • proptosis
    • decreased visual acuity
Evaluation
  •  CT scan
    • can evaluate extent of infection (whether preorbital, orbital, or abscess formation)
  • Blood cultures
Treatment
  • Emergent medical treatment
    • IV antibiotics as a hospital inpatient
Prognosis, Prevention, and Complications
  • Can spread to cavernous sinus resulting in thrombosis and meningitis
