  •  A 31-year-old woman comes to your office with complaints of ear pain that is aggravated with chewing. On exam, you note a "knocking" sound when the jaw is opened.
Introduction
  • A condition characterized by functional jaw pain without a clear etiology
  • Risk factors include
    • nocturnal bruxism (teeth grinding)
    • trauma
    • intubation
    • psychiatric conditions
Presentation
  • Symptoms
    • jaw pain worsening with jaw use (e.g. chewing)
    • pain referred to ear
    • headache
    • neck pain
    • joint clicking or locking
  • Physical exam
    • reduced inraincisor opening
    • joint clicking or locking
Evaluation
  • Imaging (e.g. CT) may be used but not commonly indicated
Treatment
  • Conservative 
    • nighttime bite guard
    • dental spliting to reduce bruxism
    • jaw stretching
    • avoidance of gum
    • warm compresses
  • Pharmacologic
    • NSAIDs
    • muscle relaxants
