Mediastinitis

Introduction
  • One of the most dreaded complications after cardiac surgery
    • most commonly present within the first two weeks after cardiac surgery.
  • Risk of mortality between 10% and 50%
  • Underlying cause is contamination from esophageal or retropharyngeal regions
Presentation
  • Symptoms
    • fever
    • tachycardia
    • increasing chest pain
    • wound drainage 
    • Physical exam
Evaluation
  • Chest xray
    • widdening of the mediastinum
Treatment
  • Surgical debridement and broad spectrum antibiotics
