Hypersensitivity to rejection is a prominent feature of avoidant personality disorder. Avoidant personality disorder is a Cluster C personality disorder. Patients are hypersensitive to criticism and rejection, socially inhibited, timid, and have feelings of inadequacy. Importantly, patients desire relationships with others, which helps distinguish these individuals from those with schizoid personality disorder. Angstman et al. review the DSM-IV personality disorders. Avoidant personality disorder has a prevalence of 5.2% in the general population and is especially common in individuals with social phobias. Both pharmacotherapy and psychotherapy may be of benefit. Oleski et al. examined the relationship between Cluster C personality disorders and major depression using data from an epidemiological survey of 35,000 patients. They found that Cluster C personality disorders predisposed patients to major depression and were likely to exacerbate major depressive symptoms in affected patients. Illustration A diagrams the Cluster C personality disorders, noting that a propensity for anxiety unites avoidant, obsessive-compulsive, and dependent personality disorders.
