You see a 6-year-old boy in clinic who presents with several days of fever and a sore throat. On physical exam he is febrile to 38.5 C, with all other vital signs within normal limits. His physical exam is notable for a bright red tongue, grey-white tonsilar exudates, a blanching erythematous "sandpaper-like" rash on his trunk (Figure A), and circumoral pallor (Figure B). Upon further review of his medical history you note that he has an allergy to penicillin, described as an itchy, red rash on the trunk and arms by the patient's mother.
