A 26-year-old pregnant woman, G2P1 at 30 weeks of gestation, presents to the urgent care clinic complaining of fever and sore throat for three days. She denies cough, headache, nausea, vomiting, and diarrhea. Her three-year-old son has been ill this last week with similar symptoms, as have several children at his daycare. She denies any past medical history and her prior pregnancy resulted in a normal spontaneous vaginal delivery at 39 weeks gestation. She works as a secretary in a physician's office and denies cigarette smoking or use of illicit drugs. Her temperature is 38.5 C (101.3 F), blood pressure 128/74 mm Hg, heart rate 88/min, respiratory rate 22/min, and oxygen saturation is 97% on room air. On physical exam, she has pharyngeal exudates and erythema with tender cervical lymphadenopathy. Cardiac, pulmonary, and abdominal examinations reveal no abnormalities, and her uterine fundal height is consistent with 30 weeks gestation.
